TERMS AND CONDITIONS
ONLINE SESSIONS
All sessions are completed online via video consultation. As a result of this, it is essential that you are not experiencing any risk when you agree to these sessions. There is a limit that can be achieved via video consultation and by signing this form you are acknowledging that you and your child/children are not at any risk of family violence, abuse of any kind, suicide or serious mental health conditions. Unfortunately, sessions may need to cease if risk of any kind does rise due to the limited capacity of online international work.
CLIENT INFORMATION
Client files are stored electronically on Health Kit. The information on each file includes personal information such as name, address, contact numbers, relevant history and details that are collected as part of the service.
PURPOSE OF HOLDING CLIENT INFORMATION
A person’s information is gathered and stored for the purpose of providing the psychological service, which includes assessing and treating the client’s presenting issue. The information is retained in order to document what happens during the session so that the psychologist can provide a relevant and informed service.
CONFIDENTIALITY
Consultations are conducted with professional confidentiality in accordance with the Code of Ethics of the Australian Psychological Society. All personal information gathered will remain confidential and secure except when:
1. Failure to disclose information would place you or another person at risk.
2. Your approval has been obtained to liaise with another professional.
3. It is subpoenaed by a court.
FEES
Full payment is required 48 hours prior to the session in order to secure your time slot.
CANCELLATION POLICY
A cancellation fee of $50 will be charged for appointments cancelled with less than 24 hours notice. *Please note that the cancellation policy will be strictly adhered to.
SUPPORT OUTSIDE OF SESSION TIMES
Email is the best way to contact me outside of your session time and I can usually reply within 24-48 hours however, this is not guaranteed. Phone calls will often be sent to voice-mail and are not a reliable way to contact me.
